CARE+CENTER LOYALTY CARD
TERMS AND CONDITIONS
The Care+Center Loyalty Card+Ruby Plan+and Diamond Plan(“The Card”) is a loyalty and promotion program offered by Care+Center, applicable only at Keralty branches in Metro Manila, including Keralty Megaclinic, Keralty Festival, and Keralty Gateway. The Card allows
Members to access consultations, services, and exclusive benefits at participating Keralty clinics. The Care+Center Loyalty Card also serves as a loyalty card, enabling Members to enjoy discounts and special offers available at Keralty branches. The Card is a convenient tool for availing Care+Center benefits and other program perks provided exclusively at Keralty clinics.
1. DEFINITIONS OF TERMS
1.1.
In this Program, the following terms shall have the following meanings:
1.1.1.
Account: Refers to the account created by Care+Center for the Member, linked to the Care+Center Loyalty Card for managing benefits and transactions related to the Program.
1.1.2.
Biological Parent: An individual who is the genetic parent of a child,
1.1.3.
Cardholder: A Member of Care+Center who holds a Care+Center Loyalty Card and agrees to abide by the applicable Terms and Conditions.
1.1.4.
Digital Card: A digital representation of the membership card issued electronically, which provides the same services and benefits as the
physical card and is accessible through designated digital platforms or applications.
1.1.5.
Inactivity: Refers to a state in which the Card has been inactive for a one (1) year, potentially limiting access to certain features or services associated with the Card.
1.1.6.
Legal Guardian: An individual who has the legal authority and responsibility to care for and make decisions on behalf of a minor or
another person who is unable to manage their own affairs, as recognized under the applicable laws and regulations.
1.1.7.
Member: An individual who has successfully purchased the Loyalty Card, completed the activation process, and is entitled to access and avail of the services and benefits associated with the card.
1.1.8.
Participating Clinics: Refers to Keralty branches in Metro Manila, specifically Keralty Megaclinic, Keralty Festival, and Keralty Gateway,
where Members can use the Card for consultations and other benefits.
1.1.9.
Program: Refers to the Care+Center Loyalty Card Ruby Plan and Diamond Plan, designed to provide Members with consultations, benefits,
and rewards through the use of the Care+Center Loyalty Card.
1.2.
References to the singular include the plural and vice versa, and references to one gender include references to the other gender. The headings used herein are for convenience only.
2. ELIGIBILITY
2.1. A person can become a member of the Care+ Center starting at the age of 15. There is no maximum age limit for membership, so as long as an individual is 15 years old or older, they are eligible to join and enjoy the benefits provided by the Care+ Center. This means that individuals of all ages, provided they meet the minimum age requirement, can access the services and advantages that come with
Care+ Center membership.
2.2. A person under the age of 15 who wants to avail the Care+ Center membership, must be accompanied by his/her biological parent or legal guardian.
2.3. If the enrollee is accompanied by their legal guardian, the legal guardian must provide their details and complete the transaction to acquire the membership card and enable access to the services.
3. CARD AVAILMENT
3.1. A person may apply for the Care+Center Loyalty Card at designated Care+Center enrollment stations located in participating Keralty clinics (e.g., Keralty Megaclinic, Keralty Festival, and Keralty Gateway) or by completing the required application form online and submitting it.
3.2. A Card Fee will apply: PHP 1,199.00 for the Ruby Plan and PHP 2,288.00 VAT included for the Diamond Plan. The fee must be paid directly at the time of application. The Card will only be issued upon successful verification of Member details and payment of the Card Fee.
3.3. The Care+Center Loyalty Card and its associated services are exclusively available to Care+Center customers who have successfully completed the payment. Any attempt to claim or use the card without fulfilling these requirements will be deemed invalid.
3.4 . For online transactions conducted through the Care+Center website, the digital version of the membership card will only be released after the transaction has been reviewed and audited for completeness and accuracy. The Care+Center Loyalty Card shall only be released through email.
4. CARD ACTIVATION
4.1. Once the Care+Center membership card is purchased, the new member may activate it by visiting the website provided on the back of the card. they may also visit the associated links in the Care+Center website for the specific Card Activation.
4.2. To complete the activation process, the member must provide the required details through the website, and all submitted data will be securely processed and protected.
4.3. Upon successful verification of the card details, the member will receive an email confirmation stating that the card has been activated.
4.4. If the card details cannot be verified, the activation process will remain incomplete, and the member will be notified accordingly through email. The services of the Care+Center Loyalty Card will not be honored until the activation is successful.
5. REFUNDABILITY AND TRANSFERABILITY
5.1. The Card is non-refundable. No refunds will be issued three (3) days after the Card is purchased or when the benefits have been redeemed.
5.2. Once the Card is activated, it becomes non-transferable and must only be used by the registered holder. The Card cannot be sold, exchanged, or transferred to any other individual or entity after activation.
6. LIABILITY AND LIMITATION
6.1. Care+Center shall not be liable for any unauthorized use, theft, or damage to the Card once it is purchased or activated. The Cardholder is solely responsible for safeguarding the Card against such risks. Additionally, the Company is not responsible for losses incurred due to delays, interruptions, or errors in service caused by factors beyond its reasonable control, including but not limited to natural disasters, technological malfunctions, or third-party disruptions.
6.2. The company shall not be held liable for any lost, stolen, or damaged membership cards and is under no obligation to replace such cards. The company’s liability is limited to the services provided by the membership card, and no other claims for damages or losses will be accepted.
6.3. Once the membership card is issued, the cardholder is presumed to be the registered member, and the registered owner assumes full liability for the card and its use.
6.4. The plan excludes the coverage of any pharmaceuticals or medical documentation in its teleconsultation services. Any requests for these items are the sole responsibility of the patient, and the patient will be required to cover the associated costs.
6.5. The 15% discount applies only to selected laboratories and diagnostic tests. It cannot be combined with other promotions unless stated otherwise. Card holders must verify eligible laboratories and tests before availing of the discount. The company is not liable for any disputes regarding discount eligibility.
7. AMENDMENTS
7.1. Care+Center reserves the right to amend these Terms and Conditions at any time. Any amendments will be communicated through the Company’s official website or other appropriate channels.
8. GENERAL TERMS AND CONDITIONS
8.1. Care+Center reserves the right to discontinue or terminate any existing arrangement with its program partners or the Program without prior notice to the Cardholder.
8.2. The Card Fee may change, subject to the approval of Care+Center and its partners.
8.3. For any concerns regarding the Account or the Card, the Cardholder may contactCare+Center through the customer service channels listed below.
8.4. Personal information updates with Care+Center will not affect the Member’s details in any external system. To update the Member’s data, the Cardholder must coordinate directly with Care+Center.
8.5. In the event that the Cardholder’s concern requires an investigation, the Cardholder agrees to provide all requested information to Care+Center and consents to the use and processing of the provided data for resolving the issue. The Cardholder also allows Care+Center to share the information with third parties if necessary for addressing the concern.
8.6. If a replacement card is required, the Cardholder may be asked to proceed to a designated Care+Center location for re-carding.
8.7. The Cardholder may visit a Care+Center partner clinic or designated office for membership-related concerns, including but not limited to:
■ Members updates
■ Benefit eligibility
■ Discount & reward
8.8. Care+Center shall not be liable for, and the Cardholder agrees to hold Care+Center harmless from, any loss, liability, or damages arising from delays or disruptions in the implementation of the Program due to force majeure, such as natural disasters, technological malfunctions, or actions by third parties, except in cases of willful misconduct or gross negligence. Care+Center will not be liable for any indirect, incidental, or consequential damages.
8.9. By using the Care+Center Loyalty Card, the Cardholder acknowledges that they have read and understood the terms and conditions of the Program and Care+Center’s policies.
8.10. Care+Center reserves the right to amend, in whole or part, any of the products, services, and corresponding terms and conditions. The Cardholder agrees to abide by any revisions, which will take effect upon notification by Care+Center. Notice of revisions may be provided through appropriate communication channels such as email, website updates, or direct messaging. The Cardholder is deemed to have accepted the revisions by continuing to use the Card or avail of the services after receiving the notification.
8.11. The Cardholder agrees that their application, use, and/or continued use of the Card or any Care+Center services is their acceptance of Care+Center’s Data Privacy Policy.
8.12. The Cardholder authorizes Care+Center to collect, use, and share Personal Data as defined under applicable laws, including customer and transaction data, as necessary to manage and operate the Program and its services. Personal data may also be shared with third parties for purposes such as marketing or compliance with applicable laws, regulations, and government requirements.

End-User Agreement and Use of Trademark
By accessing or using the Care+Center Service, you agree to abide by this End-User Agreement, which sets forth the terms and conditions governing your use of the Service. You acknowledge and agree that your use of the Service is subject to compliance with all applicable laws and regulations, as well as the terms outlined in this Agreement.
You further agree to defend, indemnify, and hold harmless Care+Center from and against any and all rights, demands, losses, liabilities, damages, claims, causes of action, actions, and suits (whether at law or in equity), fees, costs, and attorneys’ fees (including appellate level) of any kind whatsoever, arising directly or indirectly out of or in connection with your use or misuse of the, your breach of this Agreement, your relationship with or any advice provided the content or subject matter of any information you provide to Care+Center., any employee or customer service agent, or any negligent or wrongful action or omission by you or any information posted on the Site. This includes, without limitation, infringement of third-party intellectual property rights, privacy rights, or negligent or wrongful conduct.
By accessing or using the Care+Center Service, you agree to abide by this End-User Agreement, which sets forth the terms and conditions governing your use of the Service. You acknowledge and agree that your use of the Service is subject to compliance with all applicable laws and regulations, as well as the terms outlined in this Agreement.
You further agree to defend, indemnify, and hold harmless Care+Center from and against any and all rights, demands, losses, liabilities, damages, claims, causes of action, actions, and suits (whether at law or in equity), fees, costs, and attorneys’ fees (including appellate level) of any kind whatsoever, arising directly or indirectly out of or in connection with your use or misuse of the, your breach of this Agreement, your relationship with or any advice provided the content or subject matter of any information you provide to Care+Center., any employee or customer service agent, or any negligent or wrongful action or omission by you or any information posted on the Site. This includes, without limitation, infringement of third-party intellectual property rights, privacy rights, or negligent or wrongful conduct.
Use of Trademark
Care+Center retains all rights, title, and interest in its trademarks, logos, and branding materials (“Marks”). By using the Service, you acknowledge that you do not have any ownership rights to the Marks. Any use of the Marks without prior written consent from Care+Center. is strictly prohibited. Unauthorized use of the Marks may result in legal action. You agree not to copy, distribute, display, or create derivative works based on the Marks, except as explicitly authorized by Care+Center. in writing.
This Agreement represents the full understanding between you and Care+Center regarding the use of the Service and the Marks, superseding any prior agreements or understandings, whether written or oral.
