1. Is CARE CENTER CARD an HMO card?
   CARE CENTER CARD is not an HMO card and is not intended to replace an HMO membership. 
   
   It is a direct Primary Care membership that lets you take charge of your health in partnership with all Keralty branches of Metro Manila: Keralty MegaClinic, Keralty Festival and Keralty Gateway.

2.  What is Primary Care? What’s in it for a member?
   Primary care is the provision of integrated, accessible health care services by physicians and their health care teams who are accountable for addressing a large majority of personal health care needs, developing a sustained partnership with member, and practicing in the context of family and community.
   
   The care is person-centered, team-based, community-aligned, and designed to achieve better health, better care, and lower costs.

3. At what age can a member register for the CARE CENTER CARD?
    A client can be as young as 15 years old. There is no limit as to the age of a member from 15 years old and above.

4.   I have a pre-existing condition. Can I still avail of the CARE CENTER CARD?
    Absolutely! The CARE CENTER CARD is your tool in managing your health to move forward to a healthier you.

5. What do I get when I purchase a CARECONNECT Card and how much will I pay?
   At Php 1,199, you will be entitled to:

• Unlimited Primary Care clinic consultations at all Keralty Clinics and MegaClinic
• Unlimited Teleconsultation with Primary Care Physicians
• Annual Physical Examination (Basic 5: Physical exam, Complete Blood Count, Urinalysis, Fecalysis and Chest X-ray)
• 15% discount on Laboratories and Diagnostic tests
  

6. What do I get when I purchase a CARESECURE Card and how much will I pay?
    At Php 2,288, you will be entitled to:

• Unlimited Primary Care clinic consultations at all Keralty Clinics and MegaClinic
• Unlimited Teleconsultation with Primary Care Physicians
• Annual Physical Examination (Basic 5: Physical exam, Complete Blood Count, Urinalysis, Fecalysis and Chest X-ray)
• Lifestyle check-up (Lipid Profile, FBS, ECG and consultation)
• 15% discount on Laboratories and Diagnostic tests

​​​​   7. Can I substitute the APE to Pre-employment?
Yes, you may substitute the APE to Pre-employment examination covering the exact same services as described above. The Medical Certificate shall be not be included and shall be subject to applicable fees.

8. Are the Teleconsult available 24 hrs?
   The Teleconsult are available only during clinic hours.

9.   Upon registration and payment, when will I receive my CARE CENTER CARD?
   Our team will create your personalized Digital CARE CENTER CARD. Your digital card will be emailed to you within 3 working days from the date of payment.

10.    Does my CARE CENTER CARD expire?
     Yes, the validity period of your CARE CENTER CARD is one year from the date of registration, not date of payment.

11.   If I don’t have my CARE CENTER CARD with me, can I still avail of the Services?
     Yes. Upon registration, your details have already been stored in our database. All you need to do is present your valid ID for identification purposes.

12. Can I give the CARE CENTER CARD as a gift to my friends and relatives?
     We encourage spreading good health. You are free to purchase the CARE CENTER CARD for your loved ones, subject to their approval in sharing their information as required by the DOH for medical records purposes and the Data Privacy Act. 

13.    Can I use my CARE CENTER CARD at any Keralty Clinics?
    The CARE CENTER CARD benefits is applicable for Keralty branches of Metro Manila: Keralty MegaClinic, Keralty Festival and Keralty Gateway

14.     How and when can I sign up?
    You may sign up at any time online via www.keralty.com.ph or by visiting the nearest Keralty Clinic in your area.
